4DM+: A Scientifically Grounded Botanical Formulation for Metabolic Support and Healthy Glucose Balance — a Narrative Review of Gymnema Sylvestre and Pterocarpus Marsupium

Abstract

4DM+ is a proprietary botanical formulation from Biotex Life Solutions Pvt. Ltd. combining two phytochemical-rich, pharmacologically characterised herbs—Gymnema sylvestre R.Br. (Gurmar) and Pterocarpus marsupium Roxb. (Vijayasar)—for metabolic wellness and healthy glucose balance. This narrative review collates the botany, phytochemistry and pharmacology of each herb and evaluates the scientific rationale for their combined use as a foundation for future controlled clinical studies. Gymnema sylvestre leaf extract, rich in gymnemic acids, has been shown in vitro, preclinically and clinically to enhance endogenous insulin secretion, modulate beta-cell function, inhibit intestinal SGLT1-mediated glucose absorption, and improve glycaemic and biochemical parameters in human studies. Pterocarpus marsupium heartwood, a source of phenolics, flavonoids, epicatechin, pterostilbene, marsupsin and pterosupin, has shown antihyperglycaemic activity in newly diagnosed non-insulin-dependent diabetics, with GLUT4 upregulation, PPAR-γ activation and insulinotropic and antioxidant mechanisms. The two herbs act through complementary, partly non-overlapping mechanisms, providing a rationale for their combination in 4DM+. Both botanicals show favourable documented safety profiles, though robust clinical evidence for the combined formulation remains limited. The review concludes the combination is mechanistically justified as metabolic support rather than a diabetes treatment, and identifies priorities for standardisation and clinical evaluation.
