Coenzyme Q10 and Terminalia Arjuna Bark Extract in Cardiovascular Function and Metabolic Homeostasis: Mechanistic Rationale for the COQ10+ Formulation

Abstract

Cardiovascular disease is the leading cause of death worldwide, claiming 17.9-19.8 million lives annually, with a disproportionately higher burden in low- and middle-income countries. Its pathophysiology involves the interplay between mitochondrial dysfunction, oxidative stress, inflammation, endothelial dysfunction, dyslipidaemia, impaired cardiomyocyte calcium homeostasis, and the metabolic derangements of diabetes mellitus. Coenzyme Q10, a lipophilic component of the mitochondrial electron transport chain, confers cardio protection: the Q-SYMBIO international trial established that adjunctive 300 mg/day CoQ10 for 2 years reduces major adverse cardiovascular events by 50% in patients with NYHA Class III-IV heart failure (hazard ratio: 0.50; 95% CI: 0.32-0.80; p=0.003). Terminalia arjuna, the most extensively investigated cardiovascular herbal medicine in the Ayurvedic tradition, exerts pleiotropic effects via triterpenoid acids, including arjunic, arjunolic, and arjungenin, whose antioxidant properties surpass ascorbic acid; flavonoids such as luteolin and quercetin, which modulate vascular reactivity; and tannins that improve lipid profiles. Notably, Terminalia arjuna is also a natural source of CoQ10, rationalising their combination in a fixed-dose formulation. This review synthesises the preclinical and clinical evidence for the cardio protective effects of Coenzyme Q10 and Terminalia arjuna extract, focusing on COQ10+: a fixed-dose combination of 200 mg CoQ10 and 150 mg standardised Terminalia arjuna extract per tablet. By targeting six key pathways – cardiac mitochondrial bioenergetics, cardiomyocyte calcium homeostasis, endothelial function, dyslipidaemia, oxidative stress, and inflammation – COQ10+ represents a comprehensive therapeutic approach. However, while the efficacy of the individual components is established, the synergy of the combination remains to be elucidated. Consequently, assertions regarding plaque regression, restoration of calcium homeostasis, or reduction in major adverse cardiovascular events should be tempered by recognition that the fixed-dose formulation has yet to undergo clinical evaluation.
